Another Crown vic problem at low speeds

So, I fixed the other problems with the Vic... now a new one emerged...

Car is now at 165k miles still with original O2s and IACV (in case it is relevant later). It came up super clean on smog and gas mileage is decent.

So to cut to the chase...
When the car is left at idle and in D from 4-10mph it shakes... kind of feels like you're going over uneven pavement but i am positive it is due to driveline probs.

Should I wait for this to get worse or did someone have a similar problem on another car that they resolved?
 
Did you replace all the old Mercon Tranny fluid with the newer Mercon V? That is just what my old '97 GM did. Mercon V flush fixed it.
 
This car received Mercon V (fluid, filter and TC drain) every 30k miles... the most recent service was at 155k miles...

It is high up there in the miles I wouldn't be surprised if it dumped the transmission or engine at this point... but I would be a bit disappointed since this is Ford's most reliable car.
