My friend has a 2003 aviator that has black smoke on startup. What could it be, a stuck injector or bad valve cover gasket?

I went to look at it personally today and it's a little rough. Nothing reads from the ECU port when i connect my XCAL so there's definitely more going on. Gonna run a little seafoam through the tank and run some 93 in it (They put 87 in it) and see how that works.
